Before you take this test, you should know the difference between an eye check or test and a comprehensive eye examination done by an eye care professional.

In order to do this test, you will need:

PRINTER NOTE:
This test requires you to print the Near Vision Chart. The appearance of your printed chart may vary depending on the printer you are using. We have provided a ruler at the top of each chart. In order to get valid eye test results, the ruler on each printout should be approximately four inches long. Please measure to make sure.  Choose a well-lighted room away from any window glare.

 Scegliete un ambiente ben illuminato.

 Wear the glasses or contact lenses you normally use for reading.

 Utilizzate il mezzo di correzione che normalmente usate per leggere.

 Keep both eyes open.

 Tenete ambedue gli occhi aperti.

You will need someone to help you take the test. Read the sentences on the test to

your helper or write it down as it looks to you (if you do not have a helper).
Sarebbe opportuna la presenza di un assistente. Leggetegli la frase o scrivetela così come vi appare, se non avete l'aiuto. 

  Tell your helper where each opening is in the row of "C's" [or write it down as it

looks to you if you do not have a helper.]

Fate controllare all'assistente se la reale posizione dell'apertura delle "C" corrisponde alle vostre risposte (oppure, se siete soli scrivete le risposte e controllatele in seguito). 

If you did not get the sentence and all the "Cs" correct, repeat the test on another day.  

Se non riuscite a leggere la frase e le "C" correttamente ripetete il Test un altro giorno.

 If you did not get all of the sentences and all of the "C's" correct on two different days, you should arrange for a professional eye examination.

 Se non riuscite a leggere la frase e ad individuare correttamente l'apertura delle "C" in due giorni differenti è necessario consultare un Oculista.
